CERTIFICATE OF INSURANCE REQUIREMENT SHEET

The following coverages are required to be shown on a Certificate of Insurance listing Peninsula Pacific Entertainment Development, LLC (29271 Centerville Road, La Motte, IA 52054) as the certificate holder:

  • General Liability
    Minimum Coverage of $1,000,000 Each Occurrence
    Minimum Coverage of $2,000,000 General Aggregate
    Minimum Coverage of $2,000,000 Products and Completed Operations Aggregate

  • Auto Liability
    Minimum Coverage of $1,000,000 Combined Single Limit.
    Must indicate coverage for any auto – owned, hired or borrowed and non-owned vehicles.

  • Excess/Umbrella
    Minimum Coverage of $5,000,000 Each Occurrence, unless otherwise waived by Buyer

  • Professional Liability/Errors and Omissions
    Minimum Coverage of $1,000,000 Each Occurrence, unless otherwise waived by Buyer

  • Workers Compensation and Employers Liability
    Minimum Coverage of $1,000,000 Employers Liability - Each Accident Minimum Coverage $1,000,000 Disease – Each Employee
    Minimum Coverage $1,000,000 Disease – Policy Limit
    Workers Compensation - statutory limits*

    *as required within jurisdiction in which the project is being performed or services are being performed.

  • Required Workers Compensation wording:
    A waiver of subrogation has been placed in favor of Peninsula Pacific Entertainment Development, LLC; New Hampshire Group, LLC; Kansas Crossing Casino, L.C.; Legends Holding, LLC; PEN 2 Acquisition, LLC; and Cedar Rapids Development Group, LLC, as allowed by applicable law.

  • Required General Liability wording:
    Peninsula Pacific Entertainment Development, LLC; New Hampshire Group, LLC; Kansas Crossing Casino, L.C.; Legends Holding; PEN 2 Acquisition, LLC; and Cedar Rapids Development Group, LLC; and their agents, affiliates, employees, directors, owners, and officers are named as additional insured as our interest may appear on the general liability policy on a primary and noncontributory basis. A waiver of subrogation has been placed in favor of Peninsula Pacific Entertainment Development, LLC; New Hampshire Group, LLC; Kansas Crossing Casino, L.C.; Legends Holding, LLC; PEN 2 Acquisition, LLC; and Cedar Rapids Development Group, LLC.

  • Required Auto Liability wording:
    Peninsula Pacific Entertainment Development, LLC; New Hampshire Group, LLC; Kansas Crossing Casino, L.C.; Legends Holding, LLC; and Cedar Rapids Development Group, LLC; and their agents, affiliates, employees, directors, owners and officers are named as additional insured as our interest may appear on the Auto liability. A waiver of subrogation has been placed in favor of Peninsula Pacific Entertainment Development, LLC; New Hampshire Group, LLC; Kansas Crossing Casino, L.C.; Legends Holding, LLC; PEN 2 Acquisition, LLC; and Cedar Rapids Development Group, LLC.
